The following links (under-lined company names) will take you to information about the medical plans available from the insurance companies named below. The sites for American Medical Security, Assurant Health - Time, Blue Cross and Blue Shield of Georgia, Celtic Life Insurance Company, Golden Rule Insurance Company and Unicare Life & Health Insurance Company will carry you to the companies web site where you can calculate your premium for the plans available, review plan information and even complete an on-line application. The sites for the other companies not yet available on-line will provide an online form to complete and submit to receive an e-mail quote for the insurance company selected. The quotes will include instructions on how to complete and submit your application for coverage. If you have any problems with the sites or have questions, please call Forrest Veal.
12. PacifiCare - After purchasing American Medical Security, has now agreed to be purchased by United HealthCare that also owns Golden Rule Insurance Company.

HEALTH SAVINGS ACCOUNT (HSA) PLANS -
Starting January 1, 2004, all individuals covered under a "qualified" high deductible health plan (HDHP) either individual or group and, 1. not covered by any other health plan unless such plan meets the definition of "permitted insurance", 2. not enrolled in Medicare, and 3. not claimed as a dependent on another person's tax return; may establish and contribute within guidelines to a health savings account. HSA's are very much like MSA's (Medical Savings Accounts) but with fewer restrictions. No new MSA's may be established after December 31,2003 and an individual with a MSA may continue to make eligible contributions or roll the account into a new HSA.
